OD2A is guided by a data-to-action framework which uses different types of data to select, improve, and scale-up drug overdose prevention programs and policies. The Substance Use Navigator (SUN) would be an integral part of the Recovery Services team and to help our patients maintain progress, have ongoing support, and receive services in the community that we are unable to provide within the clinic walls. This role is crucial to provide linkage to and retention in care through during more high-risk times such as at hospitals, ED discharges, release from criminal justice settings, when someone is unhoused, or when someone experiences a loss/major stressor or relapse. The SUN will assist in developing overdose and relapse prevention plans and provide education to patients, family members, and the community at large. This could include distributing or helping patients locate naloxone, drug testing strips, clean supplies and syringe programs. The home care primary nurse may delegate some direct care, data collection, and nursing assessment responsibilities to other nurses and nursing assistants on the team but retains responsibility for ensuring that interventions are carried out in accordance with the participant’s care plan and the orders of the participant’s medical providers. The RN is responsible for the delivery and documentation of quality patient care in accordance with departmental policies and procedures, and in collaboration with other members of the Interdisciplinary Team. ESP RNs may provide care in the PACE Day Health Center or in participant’s homes.Essential Duties and Responsibilities:<ul><li>Assess a patient through obtaining history, performing appropriate tests, making or collecting observations from the participant and caregivers, and consulting with other members of the IDT.</li><li>Prepares and presents a patient for rapid provider review and treatment.</li><li>Arranges for or directly implements care plan interventions and testing, treatment, or other provider orders for a panel of participants.</li><li>Oversees delegated tasks to ensure completion, and monitor outcomes, and make adjustments to care plan as needed.</li><li>Safely and effectively administers medications, tests and treatments as ordered.
